I, as the parent or legal guardian, approve and give permission for my child to participate in FUN DAY 2026Â and any activities or programs offered by Unity Center. By registering for FUN DAY 2026, I acknowledge that participation in youth programs involves certain risks. I have independently reviewed and evaluated these risks and choose to allow my child to participate with full knowledge and acceptance of such risks.
